About

Home health care provides clients with a wide range of skilled care services that can be provided to them in the comforts of their own homes. We offer add-on care, and teach the patient and caregivers what they need to know about how to manage care at home. Home health care helps you get better, regain your independence, and become as self-sufficient as possible.

Services included are:

Medical Social Work

Our medical social workers provide assessments to gain a better understanding of the patient’s personal goals and what is their most pressing need. They assist patients and their family members cope with the short-term and long-term challenges by connecting them with community resources, educating on options, and facilitating discussion between patients and their families and caregivers.

Physical Therapy

Patients who are diagnosed with Parkinson’s disease, joint replacement, CHF or COPD, or other general weakness of the body can benefit from our physical therapy program. Physical therapists are available to help build strength and balance so that patients can become safer and more self-sufficient at home.

Occupational Therapy

Our occupational therapist recognizes that the most ideal place to get better is at home. Our patients and their caregivers learn to personalized adaptations for performing their daily activities more safely and independently, including strategies for moving with strength and balance, environmental adjustments, and help with assisted devices.

Speech Therapy

Our speech language pathologist will assist our patients with conditions such as motor speech disorders, swallowing difficulty, loss of language ability, and cognitive and memory conditions. They provide in-home assessments and interventions for conditions such as Parkinson’s disease, stroke, ALS, head injury, dementia, COPD or CHF.

Skilled Nursing

The main goal of our home health care nurses is to assist patients in their daily lives and ensure that they are comfortable at home and out of the hospital. Nurses evaluate each patients’ health and environment, educate patients and caregivers on disease processes and conditions to help them become more independent, and coordinate care between doctors, hospitals and other home health care team members, patients and their families.

Arranging Home Care

If a client is hospitalized or in a skilled nursing facility, you can speak to your doctor or case manager about home health care and ask for ACE Home Health & Hospice as your home health provider. We will work closely with them to provide a smooth transition to your home. If you are not in the hospital or skilled nursing facility but are mostly confined to home, you can talk with your doctor or with our admissions staff. We will work with your doctor to develop a plan for your home health care.
